ADJUNCT FACULTY ADMINISTRATIVE EVALUATION
1. Submission of Job-Related Materials
As part of the evaluation process, you may be asked to submit for review samples of job-related materials such as syllabi, class handouts, assignments, examinations, etc. The materials you are requested to submit and the date by which you are to submit these materials to your Immediate Management Supervisor are specified in the Notification of Evaluation included with these instructions.
2. Delivery of Your Completed Evaluation
When the evaluation process is completed, your written Performance Evaluation form will be forwarded to you for your review and response, either by personal delivery or by U.S. mail to an address you specify.
Please complete the bottom portion of the Notification of Evaluation form included with these instructions. Detach the bottom portion of the form and return it to your Immediate Management Supervisor within two weeks of the notice date indicated on the Notification of Evaluation form.
3. Classroom/Worksite Observation
As part of the evaluation process, a classroom/worksite observation will be conducted by your Immediate Management Supervisor or designee. The observation will be conducted on one of the alternative dates indicated on the Notification of Evaluation form included with these instructions.
4. Student Evaluations
Student evaluations will be administered as part of the evaluation process, unless the students involved are incapable of providing feedback, or where the administration of student evaluations is clearly impractical. In such case, the administration of student evaluations may be waived if you and your Immediate Management Supervisor both agree. You will be asked to leave the classroom during the administration of student evaluations.
The dates, classes or other circumstances during which student evaluations will be administered are indicated on the Notification of Evaluation form included with these instructions.
5. Performance Evaluation Form
After the classroom/worksite observation has been conducted and the student evaluations have been administered, the evaluator will complete the Performance Evaluation form appropriate to your assignment (i.e., instructor, counselor, or librarian). The evaluator will enter a rating for each performance standard, summarize the student evaluation responses, and enter an overall performance rating of either "satisfactory" or "unsatisfactory."
The completed Performance Evaluation form will be forwarded to you for your review and response.
6. Evaluation Response
Within ten (10) working days of the date of delivery or mailing of the Performance Evaluation form to you by your Immediate Management Supervisor, you must sign and date the form on the first signature line of Part III and return it to your Immediate Management Supervisor. A Aworking day@ is considered to be a weekday on which the central administrative office of the District is regularly open for business.
If you wish, you may prepare a written response to the evaluation on a separate piece of paper and attach it to the Performance Evaluation form. Your written response will be placed in your personnel file along with the evaluation form. At the time you return the evaluation form to your Immediate Management Supervisor, you may also attach a written request for a post-evaluation conference if you would like to discuss the results of your evaluation.
After you have returned the Performance Evaluation form, your Immediate Management Supervisor will provide you with a copy.
7. Follow-up Evaluations
If your evaluation results in unsatisfactory ratings or recommendations for improvement, a follow-up evaluation to assess your progress may be conducted at the discretion of your Immediate Management Supervisor.
If your evaluation results in an overall unsatisfactory rating and you have received overall satisfactory ratings for each of your previous two (2) evaluations, you may request a follow-up evaluation by checking the box and signing the last line in Part III of the Performance Evaluation form. The follow-up evaluation will be limited to assessment of the performance standards for which you have received unsatisfactory ratings.
